Sample Bequest of Remainder of Estate

No. 22-3071645, all [or state the fraction or percentage] of the rest, residue, and remainder of my estate, both real and personal. TNA – The Facial Pain Association may be contacted in care of its National Office at 408 W. University Avenue, Suite 602, Gainesville, FL 32601, 1-800-923-3608.”

Sample Bequest of Specific Amount
“I give, devise, and bequeath [$]_____ or _______[%] to TNA – The Facial Pain Association. TAX I.D. No. 22-3071645. TNA – The Facial Pain Association may be contacted in care of its National Office at 408 W. University Avenue, Suite 602, Gainesville, FL 32601, 1-800-923-3608.”
