Phase I:*

The Patient Registry, Phase I, is an effort by TNA – The Facial Pain Association to accrue statistically valid data to determine financial costs incurred in getting an accurate diagnosis of facial pain symptoms and to measure the effect and cost of treatment modalities.

All personal data will be stored on a dedicated server hosted by Acceleration.net of Gainesville, FL.

This data will not be available to any third party. The data will be used by TNA for the express purpose of advancing the cause of facial pain awareness.

* You will be required to create a separate login for the Patient Registry. If you have already registered on the TNA website, you may use the same name and password for the Patient Registry or if you prefer you may create a new one.

To start a new survey, click this button and click “new survey.” To start a new survey, click this button and click “edit survey.” You’ll need to input your last name and password used to access the survey when you registered.
